Reducing global maternal mortality - UN sustainable development goal

The Sustainable Development Goals offer openings for the international community to work together. They accelerate progress to amend maternal health for all women, in all nations, under all circumstances. Although progress has taken place in the last two decades. 295,000 women died during gestation and delivery in 2017. This number is unacceptably high.

The most common and direct causes of death are extreme blood loss, infection, high blood pressure, unsafe abortion and blocked labour. Few of them were similar to anaemia, malaria, and heart ailment. Maximum maternal deaths are preventable with timely supervision by a proficient health professional working in a supportive medium. Ending preventable maternal death must remain at the top of the global program.

About 140 million births take place every year. The proportion attended by the accomplished health labour force has increased from 58% in 1990 to 81% in 2019. It is largely due to larger births occurring at a health facility. Deaths from complications during gestation, delivery, and the postnatal period have declined by 38% in the past two decades. But at an average reduction of just under 3% per year. This rate of progression is far too slow.SDG 3 targets to ensure healthy lives and promote well-being for all ages. Its targets by 2030:

  • To reduce the global maternal mortality rate to lower than 70 per 100,000 live births.
  • To assure universal access to sexual and reproductive health-care services, including family planning, data and education, and the integration of reproductive health into civil strategies and programmes.

Videos by our doctors

When is a vacuum-assisted vaginal delivery done? | Dr. Nandyala R Padmapriya
"Vacuum-assisted delivery is a type of operative delivery. We use a vacuum in the second stage of labor whenever there is a prolonged second stage whenever there's fetal distress on the second stage or to cut down the second stage of labor in case the mother has a heart disease or severe anemia or the mother has a scar on the uterus. So these are very common indications where we use the vacuum. It is pretty safe with the advanced silicon cups vacuum is pretty safe for both the mother and the fetus if done under proper precautions. So the maternal complications could be perineal tears if episiotomies are not given properly there could be urinary retention, prolonged pain at the perineum and there could be bleeding. For the baby, the baby has a much larger exaggerated caput which is just a fluid accumulation under the scalp but which resolves spontaneously. There could be little more serious issues like capid oxum where there is a hematoma between the scalp and skull. So in these cases because of the hematoma, the baby can have long jaundice and also pain and there could be other infrequent but still very serious complications like you know hemorrhage inside the brain. That's called intracranial hemorrhage or even retinal hemorrhage"
Water Breaks Early - Best Treatment | Dr. Nandyala R Padmapriya
"When water breaks patient comes to the hospital the doctor takes a history examines the patient and first sees whether there is an immediate need for delivery that is in cases of prolapse in such cases patients will be immediately shifted to the operation theater and a C-section will be performed after checking the baby's heartbeat another cases. We also see the color of the water that if the baby has passed motion if at all baby has passed motion then that patient will be considered as high risk under continuous electronic fetal monitoring. A patient will be induced labor and we can try for a vaginal delivery if the water breakage is before 37 weeks after 34 weeks such patients are admitted and we give a dose of steroid and keep monitoring the heartbeat and also repeat a second dose after 24 hours after that seeing the status of the condition of the baby we induce the patient for vaginal delivery then if the baby is between 24 to 34 weeks all those babies definitely will survive and we have to admit the patients and we do expectant management that is we admit the patients and try to prolong the pregnancy see as much as possible and stringent monitoring of blood counts and to see for infection we give steroids and also we cover them with antibiotics. So we monitor the heartbeat in case of any issues during pregnancy minimum of at least 14 days we have the baby's weight improve if there is any infection or fetal distress then we have to decide the route of delivery based on the condition"
Is it normal to have Irregular periods during Puberty?
"Having irregular Cycles in the first 3 years after puberty is very very common. Why? This is because of the immaturity of the HPA axis. That means the brain releases certain hormones that will affect the ovary and then the ovary releases some hormones and then that will have an effect on the uterus and will cause menstruation. So when this axis from the brain to the ovary to the uterus will be immature in the initial years like in the first 3 years. So irregularity in the hormone release will be there. So irregularity in the cycle will be there. So you don't have to worry in the first 3 years post-puberty you only have to worry in this also when you have uncontrolled heavy bleeding. When the girl is losing lots of blood and becoming anemic it's affecting her general health. Then yes you should be worried about it. Otherwise, if you're getting cycles once in 3 months or once in 2 months and you have scanty flow then don't worry you just wait till the end of 3 years. Your cycles become regular as by themselves we can only reassure but yes if you are having very heavy flow you're losing a lot of blood. That's when is a point of concern"
Can a girl get pregnant if she has Irregular Periods? | Dr. Prasuna Rani P
"I can say there is a "Yes" and "No" to it. When you have irregular periods the main problem is your ovulation. If you are ovulating regularly only you can become pregnant. But when you have irregular menstruation your ovulation cannot be marked or identified. Sometimes in irregular menstruation, you will have an ovulation that the girl or the woman won't ovulate. So when there is no ovulation there is no pregnancy. When there is irregular ovulation where you cannot point it out. By chance, if you have intercourse during the time of your ovulation then you can become pregnant. So the chances of becoming pregnant will be very very low when you have irregular cycles because we cannot predict your ovulation and anovulation is a most common finding in irregular periods. So when there is anovulation you cannot become pregnant. So when you have irregular periods and you want to conceive you have to first regularize your cycles you want to have ovulated cycles then only the pregnancy possibility is there"
Causes of irregular periods after delivery? | Dr. Prasuna Rani
"After a delivery usually you won't be having cycles for a few months depending upon your breastfeeding. If you're breastfeeding your baby you may not have your cycles for 3 months 4 months 6 months or anything. But once you're just raining off your from breastfeeding the cycle irregularity can happen. Again mostly it's because of the prolactin hormone which is important for breastfeeding. When this hormone is at a higher level you may not have your Cycles or you may have transient irregular cycles. But you don't have to worry about two to three Cycles yes little disturbed and then the regular pattern sets in. So you don't have to worry when you're breastfeeding or having irregular cycles. But the only thing you should be worried about is not to have any unpredicted intercourse. Because still a possibility of pregnancy is there and for the irregular cycles post childbirth don't have to worry about waiting for two to three cycles if it's still behaving irregularly then you'll see a gynecologist"
What is Intermenstrual bleeding or Metrorrhagia? | Dr. Prasuna Rani
"Intermenstrual bleeding or Metrorrhagia is when you have to bleed apart from your regular cycle that's in between your menstruation if you're bleeding it can be a spotting. It can be bleeding for a day or two but if it happens to happen in between your cycles it is intermenstral bleeding or Metrorrhagia. The most common causes are infections like cervicitis can cause intermenstrual bleeding. Any polyps in your cervix or uterus can cause intermenstrual bleeding. This is usually seen post-intercourse. So infections and polyps are the most common reasons apart from hormonal imbalance and intermenstrual bleeding. When you present with such bleedings in between your cycles get investigated like you know get an ultrasound scan done, get your hormonal testing done. If you identify any cause treating the cause can relieve you of this. "
