I think that egg freezing is a very good idea because it gives you the power to have the fertility under your control. You can plan your pregnancy whenever you want, whenever you feel that you are ready to enjoy the motherhood and whenever you feel that your career is settled. So egg freezing is a very good option and also the success rate with egg freezing is quite good with the kind of technologies we have. Initially when IVF was new definitely success rate was not that great. But with the improvement in the IVF technology the success rate of egg freezing and conception is quite good and coming to this next part of the question where people ask how long can I freeze the eggs, yes as per the government of India we can freeze the eggs up to 10 years and once the eggs are frozen the quality remains the same till we remove it. So you need not worry about the deteriorating quality once you freeze the eggs. So we can freeze it safely up to a period of 10 years"

"That is also one of the commonly asked questions whether doctor can we go for a fresh transfer or should we go for a frozen empty transfer. So the option to go for a fresh or frozen embryo transfer relies on certain criteria like a fresh transfer. If I have to explain to you is we do the egg removal, we fertilize the eggs and in the same month in the same cycle we do the embryo transfer that is called as fresh embryo transfer. Then the other one is called as frozen embryo transfer where we remove the eggs, fertilize them, form the embryos and freeze them and after a period of time we do the frozen embryo transfer. So the success rate is almost the same fresh versus frozen the success rate almost the same but which patient should go for fresh or which patient should go for frozen lies on certain criterias like when we do the stimulation IVF stimulations. We will be monitoring the number of eggs and the response by certain hormones like estrogen and progesterones. So these will be our guiding factors to decide whether we can go for a fresh transfer or a frozen transfer. So we have seen that if the number of eggs are lots, that is around more than 12 oocytes if we get and if the estrogen value is high and the progesterone value is high. Then these patients are not perfect to go for a fresh transfer, we would ideally want to freeze the embryos and go for a frozen embryo transfer because we have seen that the complications which arises because of fresh and blood transfer when the estrogen is high or the number of eggs are more the complexion rates are very high especially the complications like ovarian hyperstimulation syndrome. It can arise when we do a fresh embryo transfer also we have seen that if the progesterones are high and we do a fresh transfer the success rate is not so good. So in such a case you would ideally want to freeze the embryos and go for a frozen embryo transfers. So if I have to broadly explain that if the patient's number of eggs are less, estrogen value is less, progesterone is well within the normal limit, then we can go for a fresh embryo transfer. But if estrogens are high, progesterone is high, the number of eggs are more then, we would ideally go for a frozen embryo transfer. The success rate of both are comparable we can go for both any either of the transfer there is no difference in the success rate. But when it comes to the risk of complications we have seen that the risk of complications in frozen embryo transfer is almost nil. The chances of complications whereas in the fresh transfer we have a complication risk of developing late ovarian hyperstimulation syndrome"

"DNA FRAGMENTATION INDEX is one of the tests we do routinely in some couples where we have had repeated IUI failure repeated IVF failure or history of smoking or history of long-term drug abuse or we feel that the abnormal sperms are very high or the female, the wife has undergone repeated abortions. First of all what is sperm DNA FRAGMENTATION INDEX? So it is kind of a sperm test which will test the integrity and damage DNA of the sperm has it and as we all know what is the DNA. DNA is a genetic material. So if the genetic material that is a DNA of the sperm is not good or fragmented or abnormal. Then obviously the chance of pregnancy is not so good and even if the couple the wife conceives with such a sperm, then the chances of abortion is high. So what do we do in such a couple is we do something called a sperm DNA FRAGMENTATION INDEX which is a sperm test and it will tell that how many percentage of the sperm are having a fragmented DNA. So we give the report in percentage twice the lesser the sperm DNA FRAGMENTATION INDEX the better is the sperm the higher the sperm DNA FRAGMENTATION INDEX or DFI. The sperm's capability of fertilizing the egg is not good. So when we use these kind of sperms in IVF even if we are doing a procedure called as ICSI, we may have fertilization failure . So ideally in such a patient what we do is we give them certain antioxidants advise them to change their lifestyles and if they have habits like smoking and all they should be quitting smoking definitely and if the sperm DNA FRAGMENTATION is very high that is more than 30 % then we do something called less directly removing the sperm from the testes where the sperm production happens. So those sperms where the sperm is freshly produced from the testes will have lower DFI or lower SPERM DNA FRAGMENTATION INDEX and we use these kind of testicular sperms for fertilizing the eggs in IVF"

"MINI IVF is also called as Low Dose Stimulation IVF or Low Stim IVF. In conventional IVF we give high dose of injections and retrieve the eggs and we have Mini IVF or Low Stimulation IVF, where low dosage of hormonal injections or tablets and hormonal injections are given. The number of eggs retrieved are not so much like in conventional IVF"

"We have seen certain couples where female has undergone family planning or sterilization, where they have undergone pregnancy and have undergone tubectomy and after some unfavorable incident they want to try for pregnancy again or patient has undergone treatment for tuberculosis which is very common in India and in such cases the tubes will not as normal as before, the tubes are scarred"

"So PGD and PGS these are the old terminologies. PGS means Pre Genetic Screening and PGD is Pre Genetic Diagnosis. So initially we used to do differently, that is screening means the patient doesn't have any history of previous abnormalities in the previous pregnancies or in the family. They just want to screen whether the embryos are genetically normal another form was PGD that is Pregenetic Diagnosis in patients who have had history of genetic abnormality in their previous child birth or there is a history of genetic abnormality in the family. Right now these two terminologies has been combined and the new terminology is Pre Genetic Testing which is PGT. So what is PGT is in an IVF where we remove the eggs fertilize with the sperm and form the embryos these embryos, we will be doing a biopsy and we test the embryos whether they are genetically normal before we transfer into the wife's uterus. So we get to know whether these embryos are genetically normal or not ideally to be done in patients who have had repeated IVF failure repeated abortions or the couple's age is more that is like 38 - 39 years of age, where we want to screen for any genetic abnormalities in the embryos or if you have a history of genetic abnormalities in the family then we can go for PGT"
